New York State's Social Care Network (SCN) initiative connects eligible Medicaid members with Health-Related Social Needs (HRSN) services, such as nutrition support, housing assistance, transportation, and care management, under the state's Medicaid Section 1115 waiver.

Where Brivon Resources Stands
Brivon Resources Inc. is seeking to participate in New York's Social Care Network as a provider of Health-Related Social Needs services. We are currently preparing for discussions with the applicable SCN Lead Entity regarding this participation.
Our goal, subject to program eligibility, applicable requirements, and SCN approval, is to connect eligible individuals with appropriate HRSN services and community resources.

Who the Program Is Generally Designed For
New York's Social Care Network is generally designed for Medicaid members who have been identified as having one or more Health-Related Social Needs. Final eligibility for any specific service is determined by the applicable SCN Lead Entity and program requirements, not by Brivon Resources.
